Date of the visit, procedure and or hospitalization A completed claim form Doctor/Medical institution bills or receipts if payment has been made In cases of surgeries not covered locally, a letter from our Chief Medical Officer stating the need for regional or international medical care Health Claim Forms: You can complete and download any of our health claim forms by clicking on the appropriate link below. Medical Claim Form (doctor visits and prescriptions) Hospital/Surgical Claim Form (surgical procedures and hospitalization) Vision Care Claim Form Dental Care Claim Form Claim Forms may also be collected from any registered doctor or at any National Caribbean Insurance Office.
